Downloadable Office Forms: These are the forms for the first naturopathic office visit and they must be completed BEFORE your scheduled visit time.

As we spend quite a bit of time for the first visit, if you are not prepared, with your forms completed we will ask you to reschedule and pay a $35 rescheduling fee. This will allow you to have the full office visit and help keep our schedule on time for others.
